(157) Incidence of Hypersensitivity Reactions During and Immediately Following Calaspargase Compared to Pegaspargase in Pediatric Acute Lymphoblastic Leukemia

Poster Abstract: Background/Rationale: Pegylated asparaginase products are an essential part of the regimens for acute lymphoblastic leukemia (ALL) in children and adolescent, and young adults (AYA). An emerging issue is the substitution of pegaspargase, dosed every 14 days, for calaspargase, dosed every 21 days, in ALL regimens. For this research, we will review the incidence of hypersensitivity reactions necessitating use of rescue medications for patients using pegaspargase or calaspargase, as this has not been documented for standard risk B-ALL or T-ALL with a Berlin-Frankfurt-Munster (BFM) backbone. There is limited availability of pegaspargase for pediatric patients; calaspargase has become the pegylated asparaginase product of choice.

Methods: This is a real-word, observational, retrospective study from December 21, 2018 to August 31, 2023 of those with confirmed B or T-cell ALL and age 1-21 years old during time of receipt of pegaspargase or calaspargase. The primary outcome is hypersensitivity reactions occurring within 3-hours of receiving pegaspargase or calaspargase. Demographic statistics, using appropriate tests such as Fisher’s exact test will be used to assess distributions of variables in the sample. There will be assessment of health professionals’ administrations of rescue medications- diphenhydramine, hydrocortisone, and/or epinephrine as a surrogate marker for the grade of hypersensitivity reactions (I to IV). Qualitative data on hypersensitivity reactions will also be culled and assessed for themes.
